Hotel Tracking Forms / Team Roster
Required for all teams at all events
Coaches, we need your help! To continue operating our events in preferred cities and facilities on desired dates we must be able to track our hotel pick-up within the city accurately. By filling out this information you give us the true economic impact our event had on the city and it's hotels and that helps us secure our dates and facilities for future years.
We sincerely thank you for taking the time to fill out this information and for staying in one of our partner hotels. By supporting the hotels that are supporting our event you are helping to keep this event in this exciting city on a great date!
Please fill out a hotel tracking form for your organization attending an event (Hotel Tracking Form).
Please fill out a team roster for each of your teams attending an event (Team Roster Form).
Liability Release & Waiver Forms
All-Star Participants are exempt from filling out medical release forms
at all Varsity Brand Events, unless otherwise stated.
2012-13 Liability Release & Waiver Form
Every participant (including All-Stars) is required to have a completed
liability release & waiver form turned in at the following 2 events.
Florida Match Play - January 26-27, 2013
Official Florida Championships - February 9-10, 2013
